Waiver

Thank you for your interest in our Summer 2024 Camp. The following form will take you through the necessary steps to sign up for Mohonk Mountain Lacrosse’s 2024 Camp. Please read and follow all instructions.

WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK 

All Participants (players, coaches and managers) must complete this form. Participants over age 18 must sign themselves. Participants under 18 must have the signature of their parent or guardian.

Return completed form to Mohonk Mountain Lacrosse as requested. In consideration of being allowed to participate in any Mohonk Mountain Lacrosse camp, related events and activities, we the undersigned: 

1. ACKNOWLEDGE that we have read this WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK and we fully understand its terms and agree to its terms, understanding that we have given up substantial rights in signing it. 

2. ACKNOWLEDGE and fully understand that each participant will be engaging in activities that involve risk of serious injury, including the risk of death from any cause both on and off the field of play and the risk of damage, loss or theft of property. Insurance is provided through participants USA lacrosse membership, that is required upon registration.

3. Nevertheless, ACCEPT those risks and assume all foregoing risk, whether foreseeable or not, and all responsibility for any losses, costs, expenses, liabilities and damages that may be incurred as a result of or in connection with my, or my minor child’s, participation in any way in any activity. 

4. RELEASE, WAIVE, and COVENANT NOT TO SUE Mohonk Mountain Lacrosse, USA Lacrosse, and New Paltz Parks & Recreation District and their respective affiliated entities, officers, directors, members, managers, employees, agents, representatives and contractors, and all officials, sponsors and vendors of the camp from demands, losses or damages on account of injury incurred as a result of participation in the activities of the camp, including travel to and from said camp.

5. ACKNOWLEDGE that rain dates for the camp will take place on August 17^th and 18^th, Make up days will take place at the original venue, 241 Libertyville Road, New Paltz, NY, 12561. If you are unable to attend either of the make-up days, there will be no refunds.

MEDIA RELEASE

In consideration of my, or my child or ward’s participation in a Mohonk Mountain Lacrosse interaction, I, grant Mohonk Mountain Lacrosse and its employees, contractors, volunteers, representatives, directors, and officers (collectively, “Mohonk Mountain Lacrosse) the license to use my name, or my child’s likeness, image, photograph, voice, video, athletic performance, biographical, and other information (collectively, “Likeness”), in any media platform or format whatsoever, and to distribute, broadcast and exhibit these without charge, restriction or liability and such shall be the property of Mohonk Mountain Lacrosse in perpetuity. I hereby release and discharge Mohonk Mountain Lacrosse from any and all claims, demands, or causes of action arising out of or in connection with the use of my, or my child’s likeness for the purposes described herein.
